According to the Attorney General’s legal papers, the Director of Nursing at Woodmere Rehabilitation and Health Care Center ordered one of the nurses to "fill in the gaps" in the patient log although she had not worked on the relevant days at issue, and also told nurses to "go home" in an effort to evade the Department of Health investigator, court papers said.
